For medical reasons, I consent:
I am not currently pregnant or breastfeeding.
I have not been on antibiotics within the past 2 weeks for any type of infection.
I have not had any facial surgery in the past 12 months.
Booking Terms – Please read in full.
I consent that I am over 18 years of age. It is a legal requirement that we see valid ID before treatment. Please upload a copy of your ID to the consent form that will be sent to you on booking.
I understand that my booking fee is in exchange for an allocated and reserved time slot of my choice, and that these fees are non-refundable. Booking fees may be transferred to a different date on one occasion if we are given a minimum of 48 hours’ notice. Further reschedules require a new booking with a new booking fee.
We cannot guarantee that we will be able to see you if you arrive more than 10 minutes late to your appointment. You may be refused by the clinic and lose your booking fee.
We are not insured to treat children on the premises. If you arrive with anyone under the age of 12, we will not be able to proceed with treatment and you will lose your booking fee.
I understand that before and after photographs must be taken of me for my own clinical records.
I understand that it may take multiple appointments to achieve my desired result.
I understand that dermal fillers, anti-wrinkle injectables and skin boosters are all temporary and require maintenance.
